| 2 Application for the reinstatement of the right to study If you lost your right to study because you failed to register for the academic year, you may apply for the reinstatement of your right to study. |
| 3 Termination of studies If you wish to terminate your studies without graduating you must submit this form. You will lose your right to study immediately. |
| 4 Individual study plan You must submit this form if you apply for student financial aid for the summer (June-August) or certain exchange studies. |

| 6b Application for accreditation of study attainments - informal and non-formal competence If you think you have acquired competence that is part of your degree by some other means than university studies, you may apply for accreditation with this form. |

Exception: the office is open 10-12 on the Fridays that are deadlines for handing in applications of degree certificates.
Administrative tasks are carried out when the office is closed.
